    Case:

    5yr old Thoroughbred Gelding - **Name**

History Provided:

    We have been given no Vet reports, Scans or Operation Notes but have been advised that **Name** has been raced only once and that he broke down lame in the Stifle Joints. Also that he went lame on two previous occasions under 100% training loads.

    We have also been advised that operations have been performed on both stifles removing "bone cysts"

    **Name** has been referred to us with instructions to treat with herbal formulations to repair any residual problems in the stifle, and to reduce the likelihood of arthritis setting in, following the history of lameness and the operations.

Case Notes:

    Our Observations:

    We find **Name** to be a big well formed heavy type of gelding, carrying resting condition and apparently not lame at present. Leanne has started swimming exercise only in the few days she has had him, to build up a little fitness prior to starting work on the ground next week.

    **Name**'s history leads us to suspect that rapid growth or structural stress early on, could have allowed an exaggerated range in the stifle flexion, with resultant abnormal loads leading to bone abnormality, and presenting as cysts.

    Additional History and Information:

    We should like to be provided with a full medical and training history for **Name** including a history of medicines prescribed and copies of any reports accompanying scans or surgical procedures. This information will be kept with his files and returned at the end of his rehabilitation program.

    Suggested Course of Action:

    Although we have scant information at this point, the fundamentals of the case remain clear, and are sufficient to start the basic treatment and to formulate a first approximation of the retraining plan.

    Treatments:

    A modification to our standard Ligament and Bone rehabilitation program, is prescribed as follows;

    One cup of herbal mixture, twice daily mixed with feed along with a daily application of a healing oil massaged lightly into the inside of the stifle joint.

    The aim of this program is to stimulate the normalization of bone tissue in the stifle, while at the same time re-tensioning the ligaments involved with the joint. The same treatment will begin a process of normalizing scar tissue at the site of the operation and reduce the likelihood of arthritic changes occurring in the future.

    Variations on this treatment will be carried right up through preparation, and for a period of several months at full workload, to ensure that the repair work will support the demands of regular racing.

    Training:

    We recommend specific groundwork in a small round yard to start with, twice a day on a lunge lead at a relaxed trot with the head carried low. This would begin with just 5 minutes each side and lead slowly up to about 15 minutes per side, as he becomes fitter and more relaxed and comfortable.

    This trot work will later be combined with canter and walk, and will result in improved musculature along the back, improved flexion both ways, and the rebalancing of his movement generally.

    These exercises will support the healing of bone damage, assist with the re-tensioning of the stifle joint ligaments, and strengthen the musculature generally in the hindquarters.

    For the first two weeks of slow buildup of work we recommend **Name** not be ridden and that these two daily sessions along with his swimming, form the main part of his exercise.

    We also recommend that he be allocated space to move about and exercise freely outside of his box for several hours each day.

    At this stage we expect at 2 weeks, to introduce ridden work in addition to the round yard work. This will include figure 8 work, to continue the development of fitness and strengthening the hind quarteders, under a rider.

Program Outline:

    As a preliminary program we are expecting around 6 weeks buildup in fitness before introducing any track work. After this we expect that the ligaments and bones will be well on the way toward healing and will benefit from the increasing loads of more standard preparation training.

Reassessment & Reporting:
    I will provide to the owner, trainer, and consulting Vet, regular reports on the treatment and progress. In these reports we will re-assess treatment, and update the program to predict more closely when **Name** will be fit and sound for racing.

    I will visit **Name** at least twice per week and write up our first progress report and revised recommendations, after two weeks.

    Comprehensive reports will be prepared at monthly intervals thereafter and all reports will be faxed or emailed out to you direct.
